Uninsured Services

The following services are not covered by Alberta Health. LA Medical Clinic is required to recover the costs from our patients.

14. Audiogram **$50**
15. Medical Records Request **$50**
16. Insurance Form/ Form Completion **$30** for one page
**$25** for each additional page
17. Rapid Strep Throat Test **$20**

Processing: Reports require two weeks for completion and can be delayed by failure to complete the patient details and authorization in full.
